Supervısed Approach Cognitive Behavioral Therapy Approach Case Formulation: Presentation of Generalized Anxiety Disorder

Abstract

The aim of this case report is to examine the psychotherapy process of a 19-year-old unemployed female client presenting symptoms of Generalized Anxiety Disorder according to DSM-5-TR diagnostic criteria within the framework of Cognitive Behavioral Therapy (CBT). In this study, the client’s intense anxiety symptoms, dysfunctional cognitive distortions, avoidance behaviors, and their effects on daily functioning were evaluated. The therapy process aimed to identify negative automatic thoughts, restructure dysfunctional cognitions through cognitive restructuring techniques, and reduce behavioral avoidance patterns maintaining anxiety symptoms. At the beginning of the therapy process, psychoeducation regarding Generalized Anxiety Disorder was provided to the client, followed by cognitive and behavioral interventions. During the sessions, thought patterns increasing anxiety levels, intolerance of uncertainty, feelings of inadequacy, and avoidance behaviors were addressed. In addition, breathing exercises, relaxation techniques, and behavioral activation interventions were implemented to improve the client’s coping skills. Throughout the therapy process, a decrease in anxiety symptoms, improvement in daily functioning, and partial reduction in social withdrawal behaviors were observed. Furthermore, the client demonstrated increased cognitive awareness and began to evaluate negative automatic thoughts in a more functional manner. The psychotherapy process was conducted under supervision, and both case formulation and therapeutic interventions were regularly reviewed during supervision sessions. This case report supports the effectiveness of Cognitive Behavioral Therapy in reducing symptoms of Generalized Anxiety Disorder and improving the client’s functionality. In addition, supervision support is considered to contribute significantly to the structuring of the therapy process and the development of therapeutic interventions. Future studies conducted with different sample groups may contribute further to the literature.
